What happens to me once I am discharged from hospital?
This is a question that many people ask. Even relatives are often concerned as to what happens next. Social services will not leave you alone with these questions. At your request, we are here for you in a personal discussion. These consultations are naturally confidential and free of charge.
Range of services
Social services supplements the nursing and medical range of services offered by the hospital by working closely with all professional groups who take care of the patients. Social services provides advice when it comes to family, professional and financial difficulties and develops solutions possibilities in joint discussions. We put people in touch with nursing homes, outpatient nursing services and day-care centres and organise the provision of aids for home care, if necessary. In addition, we also put people in touch with advice centres which focus on addiction, migrants, people with psychological illnesses, nursing relatives as well as to official institutes such as the social welfare office, health department and youth welfare office. We also assist patients and relatives in applications to the nursing insurance institution, the relief organisation and social authorities.
