
Acute therapy is part of a model project which is supported by the federal government as well as the Free Hanseatic City of Hamburg. The concept explicitly envisages an integrative collaboration of the various departments at the Wilhelmsburg Groß-Sand Hospital.
In this way, a patient within the framework of a serious acute illness can, for example, initially receive internistic treatment in our intensive care unit. After the treatment in the intensive care unit, further treatment in the acute geriatrics is possible, which includes intensified rehabilitation measures.
In particular where neurological-geriatric rehabilitation is required, patients can be transferred at an early stage to the department for geriatric early rehabilitation within the scope of a therapeutic step-by-step plan, or - if full inpatient care is not longer necessary - to the outpatient clinic. Rehabilitation measures are a priority there. This ensures an optimal appropriate and linked treatment from admittance to discharge.
Naturally, a transfer to acute geriatrics from other departments of the hospital is also possible. Even external can be transferred to our acute geriatrics department directly from a registered doctor in private practice or by another hospital.
There is close collaboration between the patients' general practitioners, neighbouring outpatient nursing services, outpatient therapeutic and social institutions, hospice services as well as old-age homes and nursing homes.
Numerous patients living alone are able to be discharged home - despite their need of care - often with the help of one of the emergency systems installed in their flat. This system allows the patients to get help quickly, for example at night or after a fall.
The range of services offered by our outpatient clinic and our department for geriatric early rehabilitation as well as the neighbouring old-age day facilities and other social offers for senior citizens in the South Elbe Region, provides the possibility of further rehabilitative and social reactivation even after dismissal. Here there is close cooperation with the relevant facilities via the medical services and our social workers.
